NPC Melt Rune have bug after update 7 sep 2016

Dear GM,

i melted cata 16x+5 at zaid
it showed possible success rate
blue rune + red rune + black rune + white rune
is 100%

but after melt it's fail !!?

what's going on?

I think refining is messed up. My friend can produce random equipment using Laxative Bomb Manual.
