Forge Succes "only 3 items" ?

other People and me have the same Problem...have forged 6-8 Items but only 3 succes other items all destroyed what is the Problem ?

rare, unique or ancient?
Failed one rare part, but succeeded 4 others and succeeded on going ancient (did use pres stone on the latter just in case). This for natu gauntlets (finally got them after 3 weeks..)

Try to switch up on dif. chars. I forged a Tali bow, tali chest and tasset with like 4 fails total.

Forged full set Natu and Muitak without pres stones and got 0 fails. I always forge 3 parts and then random **** til it fails
Then again the parts I actually want.
