FTTC + ADSL2+/VDSL/VDSL2 (which one?) or Cable?

Don't think I have ever seen a DSL setup that uses a cable-centric DOCSIS 3.0 modem ;)

Yes, Cable can give up to about 250/20 Mb/s U/D on DOCSIS 3
