Use catalyst (high lvel equip but unused one)
Go to zaid and you will find NPC who will melt your equip for rune stone.
Beware they cost some money and can fail too.
And also must at least +4

Higher level equip mean better chance.
That's why people trading Cata 20x

You can have stones by Melting just like ashmstrike says :)
errrr GMs alos uses it as a reward it during hide and seek.
