Lvl 62 Quest, The Honorable Death is not being given out by NPC Rima Regenon at Vena. Would really like to do that quest for the good accessory. Is that quest bugged ?

No, it gives it out, only after you complete lvl 40 ses quests+the one lvl 45 main story quest (if you mean 15 stats 200hp ring). Only after you complete those, it will give you The Honorable Death quest
