I was wondering if upgrade my armour/weapon to +5(In Jin server) and if it fails, would my item disappear? And if I want to +6 the armour/weap I will need the enhanced abradant stone?

no dissapear , but if u want ehance +6 up u need the stone if u dont want broken :)
